    Case Study #1: Healthcare Case

Anika and Daniel, two geriatric nurses, are discussing a patient with worsening dementia. Anika insists they should continue following the doctor’s orders to use a mixture of cholinesterase inhibitors to treat the patient. In her view, because the doctor’s orders are based on peer-reviewed literature and because rigorous testing has shown that these inhibitors help prevent the breakdown of acetylcholine (a chemical that helps brain cells communicate), these orders are likely to help their patient. Daniel judges this peer-reviewed research and testing as unreliable merely because he believes that mainstream treatments doctors recommend never work. He is frustrated by the lack of improvement so far and thinks they should try something else.

Daniel then shares that he recently performed an online search on “alternative approaches to cholinesterase inhibitors” and found an online health forum where caregivers claim that exposure to certain frequencies of sound helped memory retention. He shares a story about his grandmother who listened to classical music and seemed sharper afterward. He then points out that a well-known wellness influencer recently talked about the effectiveness of a new sound therapy device that emits healing frequencies, and that many of the influencer’s audience members have ordered the device already. He suggests buying one of these devices for the patient.

When Anika expresses doubt, Daniel responds, “Nurses should be willing to try anything that might help. Since this might help, we must give it a try. Also, you are from the Midwest, so it is clear that your medical reasoning should not be trusted.”

    Overview

The purpose of this assignment is to write a problem statement that focuses on a potential topic for your dissertation research. A problem statement narrowly defines your topic area to a specific problem or issue. In this case, you are taking the next step in developing your dissertation topic that was the basis of your assignment in Week 4—Research Topic Investigation. The idea is to take what you learned in completing that assignment and what you gained from your instructor’s feedback to develop a more formal, concise, and specific statement of a precise problem that can be researched. The skills involved in writing a problem statement will serve you well throughout your program and are particularly important in the dissertation process. They are critical skills needed to become a competent doctoral researcher in psychology.

A research problem is a concern, issue, or controversy that you investigate. The problem may come from the need to extend research into an area not yet studied. Many times, the specific problem or issue is identified at the end of another research study where the author calls for additional research. Regardless of the source of the problem, a researcher needs to have a clear idea of the goal: the answers that are sought and the specific questions that need to be asked.

This assignment will not be your final statement of this problem. The mini literature review you will complete in Week 8 may cause you to revise your statement to some extent, based on what you learn. However, this problem statement should be more clearly defined and more precise than the assignment you submitted two weeks ago. You are familiar with the names of the researchers who have already studied your topic area. You have located several literature resources—articles, books, and summaries—that provide some of the existing knowledge on that topic area. In the discussions and writing assignment in Weeks 3 and 4, you may have already begun the process of summarizing what is already known in the topic area. When you write your problem statement, rely on this work to help you articulate a focus for your research—a concise summary of what is already known and a clear statement of what you want to find out. What questions do you want to answer?

Writing a clear problem statement requires some significant thinking; consider this assignment as a learning experience, one that helps you hone your thinking in preparation for attending the residencies.

    Instructions

In a 1–2 paragraph problem statement (180–250 words):

    ●      State the research problem clearly and succinctly.

    ●      Summarize what is already known and the gap in the literature you perceive to exist.

    ●      Justify very briefly the need to study the problem by presenting a couple of reasons why the issue is important.

    Additional Requirements

Your assignment should meet the following requirements:

    ●      Written communication: Include written communication that is free of errors that detract from the overall message.

    ●      APA formatting: Format resources and citations according to APA style and format guidelines.

    ●      Number of resources: Include a minimum of two resources.

    ●      Length of paper: Write 1–2 pages, double spaced, not including the title and references page.

    ●      Font and font size: Use Times New Roman, 12 point.

Review the Problem Statement Scoring for more information on how you will be assessed.

Submit your assignment as a Microsoft Word attachment in the assignment area.

    Competencies Measured

By successfully completing this assignment, you will demonstrate your proficiency in the

following course competencies and scoring guide criteria:

    ●      Competency 1: Apply advanced critical thinking skills.

    ○      State a specific problem or issue to be investigated in your specialization of interest, including variables of the research.

    ●      Competency 2: Apply research processes and standards and the technical skills of a researcher in the field of psychology.

    ○      Explain specific research of what is currently known in the field on the selected topic.

    ●      Competency 3: Analyze and apply professional ethical standards in psychological research and practice.

    ○      Justify the need to study the specific issue and how the findings will ethically advance practice in the field.

    ●      Competency 4: Develop a professional identity as a doctoral-level psychologist.

    ○      Describe a clear gap (what is not known) in the current research on the selected topic to be addressed.

    ●      Competency 5: Convey purpose, in an appropriate tone and style, incorporating supporting evidence and adhering to organizational, professional, and scholarly writing standards.

    ○      Convey purpose in a well-organized text, incorporating appropriate evidence and tone in grammatically sound sentences.

    ○ Apply APA style and formatting to scholarly writing.

    Week 4 Assignment

    Healthcare Finance

To deliver healthcare services, an ACO needs to create a competent workforce that can provide health care to the patients that are in the ACO. Some integrated ACO entities already have employed physicians and mid-level providers that receive salaries from the organization, whereas other non-integrated ACO entities need to invest in external relationships with independent physicians and other providers. Since ACOs do not have managerial control over non-employed providers, additional controls and incentives may be necessary to ensure that efficiency is maximized. ACOs also need to consider the number and mix of specialists (e.g., family practice vs internal medicine) and mid-level providers (e.g., physician assistants and nurse practitioners) in delivering primary care services that can promote value. Maximizing value requires appropriate workforce decisions on supply and incentives to encourage providers to minimize costs. For this Assignment, you will examine the Week 4 Supply of Workforce Template and consider the efficiency in the supply of health services.

    To prepare for this Assignment:

    • Review the scenario and Week 4 Assignment document provided to you by the instructor. Based on the financial data, conduct a financial projection (revenues, expenses, and profit) that analyzes the efficiency of the supply of health services.
    • Download Week 4 Supply of Workforce Template.

Submit the template provided with calculations and a brief 1-page document, to address the following:

    • Propose recommendations on the following:
      • Number of physicians and nurse practitioners needed
      • Reimbursement method: salary or fee-for-service
      • Recommendations for financial incentives to address the challenges of supplier-induced demand
      • Recommendations for how to ensure efficiency
    • Interpret the net profit from the ACO contract based on your recommendations.
    • Explain the rationale behind your recommendations, including the impact made by your financial calculations.

Please see attached for week four assignment.  You must complete both the salary and FFS while the recommendations can be a one-page summary.

    Week Four Supply for Healthcare

Supply takes on a prominent role in health economics because of the unique nature of what is being supplied in healthcare. Moreover, supply in healthcare is subject to many influences that are not as evident in other kinds of markets. The production of healthcare providers is more regulated and requires far more resources than the production of taxi drivers or florists, for example. Patients and family members, communities throughout the country, doctors, nurses, and providers are all grappling with the reality of the depletion of the physician workforce in our nation’s healthcare system. The workforce shortage has been increasing since before the COVID-19 pandemic—accelerating at an even more alarming rate thereafter. By 2034, according to the Association of American Medical Colleges, the demand for primary and specialty care physicians will exceed supply by a range of 37,800 to 124,000 physicians (USA Today, June 29, 2023). This will require providers to carefully consider utilization management controls.

This week, as you explore the supply of health services, you recommend strategies for reimbursement and utilization management. You also examine efficiency in the supply of services and challenges of supplier-induced demand.

    Week 4 Assignment

Assignment: Supply of Primary Care Workforce

Scenario: Your hospital has an opportunity to participate with an Accountable Care Organization (ACO) contract with 50,000 ACO patients.  The ACO patient population expects about 5,000 avoidable hospitalizations per year with a potential savings of $50 million, which will be shared 50% revenue to the insurer and 50% revenue to the hospital.  The ACO patient population will require an average of 6 office visits per year, and 300,000 total office visits will need to be supplied by your primary care workforce.

    To prepare for this Assignment:

Based on the scenario and financial data provided by the course instructor, conduct a financial projection (revenues, expenses, and profit) which analyzes the efficiency in the supply of health services.

    The Assignment:

In a 2- to 3-page Word document that includes tables and/or calculations, make recommendations on the following: 1) number of physicians and nurse practitioners; 2) reimbursement method: salary or fee-for-service; 3) recommendations for financial incentives to address challenges of supplier induced demand and ensure efficiency. Interpret the net profit from the ACO contract based on your recommendations. Support your recommendations with rationale, including how the financial calculations impacted your recommendations.
